Services
Carolina Hospice Care is a state licensed healthcare agency specializing in palliative care for the terminal ill and those with life limiting conditions. Unlike traditional healthcare, provided in a hospital or clinic setting, our services are also provided in your home. Our staff comes to you.
Palliative care focuses on comfort and symptom management. The difference between palliative care and curative care is that rather than “cure” a disease process, many of which, have no cure, our nursing staff will monitor your condition at least on a weekly and as often as daily when your condition warrants to ensure your medications are having the desired effects, keeping you comfortable. Because we specialize in palliative care, our staff can anticipate your healthcare needs and act on your behalf with your primary care physician. For example, many patients with dementia, chronic heart or chronic respiratory conditions may qualify for hospice care.
Unfortunately, there isn’t a cure for dementia conditions like Alzheimer’s. The medical community can only offer medications that can slow the progression of the disease in the initial stages. Dementia not only affects one’s memory, but in the later stages, it affects the ability to eat and walk as the person become totally dependent for all aspects of care. Persons with end stage dementia may qualify for hospice. A person with a heart condition, who is taking medications and still having symptoms or “exacerbations” like chest pain, shortness of breath, or swelling; and whose only options are heart surgery or a heart transplant, may quality for hospice care. A person with COPD or other chronic respiratory conditions, who has frequent visits to the emergency room because of difficulty breathing, may qualify. And of course, there is the traditional hospice client. Those with cancer for whom chemotherapy and/or radiation has failed or is just not an option may qualify for hospice care.
Palliative care also focuses on supporting the caregiver. In addition to our nursing staff, the Carolina Hospice Care team includes chaplains, counselors, volunteers, social workers, home health aids and our medical director. Our staff collaborates with your primary care physician to provide you with the services you need to remain at home in comfort including medication and medical equipment. Not only do we provide care to the ill, but we also empower the family and act as an advocate for the ill. Our team meets regularly with you and your family to discuss your concerns and our plan of care. We provide your caregivers with education about the disease process and strategies for keeping you comfortable. We make sure they know what medication to give you and why. But more than that, we understand how hard it is for the caregiver. We understand that you are a more than an illness and how your illness affects the whole family. We provide support to the caregiver with home health aids who can help with the tasks of caring for the ill, volunteers with a variety of skills, and professional chaplains and social workers who understand the stress of being a caregiver and dealing with a terminal illness. Our team is available 24 hours per day so we are only a phone call away when you need help.
Carolina Hospice Care provides palliative end of life care, as do many other hospices; however, the difference with Carolina Hospice Care is our philosophy of care. We are locally owned and managed. Our staff takes the time to get to know you and your family. Our executive staff participates in care plan discussions to ensure we are providing the best possible care and customer service. In addition, we are the first hospice to use Consumer Directed Care, a new approach to healthcare, in conjunction with the University of South Carolina. The idea of Consumer Directed Care is our staff empowers you to make healthcare decisions regarding your terminal illness and we advocate on your behalf with your primary care physician. We personalize our care to meet your needs. Our team works collaboratively with you to schedule our visits at your convenience.
